Students requesting the Scholarship to pay for a failed or repeat course must complete the following form and attain the signature and recommendation of their academic Chicago Early Learning Workforce Scholarship (CELWS) advisor. The program may pay for a required class a maximum of two times. After the second time, it is the student’s responsibility to pay out of pocket for the course.

Students whose GPA has fallen below a 2.0 may request continued support from the Scholarship, with the recommendation from their CELWS academic advisor. If student’s GPA is below 2.0 for two consecutive semesters/terms, the student’s Scholarship may be withdrawn.
